Player Releases
If you require a player release from the Connecticut RiverHawks, please submit the following information to Erica Siegel who will submit the requst to the CHC:
- Child/Player Name
- Address
- Phone Number
- Birthdate
- Name of the new hockey program
In order for the Connecticut RiverHawks to submit the request, all financial obligations must be resolved.
